the provisions of the said Act, unless the name of such child shall have been returned to the said Central Vaccine Board, in some register, as having been sufficiently vaccinated, or unless the legally qualified medical practitioner having vaccinated such child, or a District Vaccinator, shall certify under his hand that such child has been sufficiently vaccinated; and every legally qualified medical prac- titioner, or District Vaccinator, giving a wilfully false certificate, ss
