to any penalty under section 21 of " The Vaccination Act, 1882," in respect of any child born before the passing of this Act if before the thirty-first day of December, one thousand nine hundred and seven, he makes a declaration before a Justice of the Peace, in the form of the Schedule to Act No. 761 of 1901, that he conscientiously believes that vaccination would be prejudicial to the health of the child in respect to which the declaration is made.

3. Upon receipt by the Vaccination Officer of any declaration Vaccination odcer to

give or send by post to the person making such declaration an acknowledgment of the receipt thereof under his hand or the hand of his clerk.
