MALIGNANT NEOPLASM OF THE ENDOMETRIUM

Statement of Principles

concerning

MALIGNANT NEOPLASM OF THE ENDOMETRIUM

1.         Being of the view that there is sound medical-scientific evidence that indicates that malignant neoplasm of the endometrium and death from malignant neoplasm of the endometrium can be related to operational service rendered by veterans, peacekeeping service rendered by members of Peacekeeping forces and hazardous service rendered by members of the Forces, the Repatriation Medical Authority determines, under subsection 196B(2) of the Veterans’ Entitlements Act1986 (the Act), that the factors that must as a minimum exist before it can be said that a reasonable hypothesis has been raised connecting malignant neoplasm of the endometrium or death from malignant neoplasm of the endometrium with the circumstances of that service, are:

(a)       being nulliparous immediately before the clinical onset of malignant neoplasm of the endometrium, unless the nulliparous state is attributable to treatment with the combined oral contraceptive pill; or

(b)       suffering from Stein-Leventhal syndrome before the clinical onset of malignant neoplasm of the endometrium; or

(c)       being treated with the drug tamoxifen before the clinical onset of malignant neoplasm of the endometrium; or

(d)       undertaking a course of hormonal replacement therapy before the clinical onset of malignant neoplasm of the endometrium; or

(e)       undertaking a course of therapeutic radiation involving the pelvic organs before the clinical onset of malignant neoplasm of the endometrium; or

(f)        being obese for at least ten years before the clinical onset of malignant neoplasm of the endometrium; or

(g)       inability to obtain appropriate clinical management for the malignant neoplasm of the endometrium.

2.         Subject to clause 3 (below) at least one of the factors set out in paragraphs 1(a) to 1(g) must be related to any service rendered by a person.

3.      The factor set out in paragraph 1(g) applies only where:

(a)       the person’s malignant neoplasm of the endometrium was contracted before a period, or part of a period, of service to which the factor is related; and

(b)       the relationship suggested between the malignant neoplasm of the endometrium and the particular service of a person is a relationship set out in paragraph 8(1)(e), 9(1)(e), 70(5)(d), or 70(5A)(d) of the Act.

4.      For the purposes of this Statement of Principles:

“a course of therapeutic radiation” means one or more fractions (treatment portions) of ionising radiation administered with the aim of achieving palliation or cure with gamma rays, x-rays, alpha particles or beta particles;

“baseline weight” means the weight level which was being maintained prior to the effect of the particular factor specified;

“being obese” means an increase in body weight by way of fat accumulation which results in a Body Mass Index (BMI) of 30 or greater.

The BMI = W/H2 and where:

W is the person’s weight in kilograms and

H is the person’s height in metres;

“combined oral contraceptive pill” means a compound containing both oestrogen and progestogen taken in order to block ovulation and prevent the occurrence of pregnancy;

“hormonal replacement therapy” means the addition of sequential and unopposed oestrogen to offset a hormone deficiency following surgically induced or naturally occurring menopause;

“ICD code” means a number assigned to a particular kind of injury or disease in the Australian Version of The International Classification of Diseases, 9th revision, Clinical Modification (ICD-9-CM), effective date of 1 July 1996, copyrighted by the National Coding Centre, Faculty of Health Sciences, University of Sydney, NSW, and having ISBN 0 642 24447 2;

“malignant neoplasm of the endometrium” means a malignancy of the inner mucous membrane that lines the uterine cavity, attracting ICD code 182.0;

“nulliparous” means having never given birth to a viable infant;

“Stein-Leventhal syndrome (also known as polycystic ovary disease)” means a clinical symptom complex associated with polycystic ovaries and characterised by oligomenorrhea or amenorrhea, anovulation and hirsutism, attracting ICD code 256.4;
