2 Poisons Amendment (No. 2) No. 107, 1994 ˙ Short title 1. This regulation may be cited as the Poisons Amendment Regulation (No. 2) 1994 . ˙ Regulation amended 2. This regulation amends the Poisons Regulation 1973 . ˙ Amendment of s A1.01 3. Section A1.01— insert — ‘ “approved form” means a form approved by the Chief Health Officer under section A1.02; ’. ˙ Insertion of new s A1.02 4. After section A1.01— insert— ˙ ‘Forms ‘A1.02.(1) If a provision of this regulation requires a person to use an approved form, the Chief Health Officer must approve a form for the purpose of the provision. ‘(2) A person may ask the Chief Health Officer for a document setting out an approved form. ‘(3) The Chief Health Officer must comply promptly with the request. ’. ˙ Amendment of s A2.06 5. Section A2.06— omit— ‘Schedule 5 1. after the entry ‘Warfarin’, insert ‘ZINEB’. ’.

After section A6.04— insert— ˙ ‘Clozapine ‘A6.04A.(1) A person must not prescribe, dispense, sell, lend, give away, supply or use clozapine for human therapeutic use unless— (a) the person’s name appears in the Register of Specialists, Queensland as a psychiatrist; or (b) the person is a registrar in psychiatry working directly under the supervision of a specialist psychiatrist; or (c) the person has the written approval of the Chief Health Officer. ‘(2) However, a pharmacist or a doctor practising pharmacy with the approval of the Pharmacy Board, may dispense clozapine under the written prescription of a person mentioned in subsection (1). ‘(3) In this section—
