This by-law commences on 1 January 1996. ˙ By-law amended 3. This by-law amends the Pharmacy By-law 1985 . ˙ Omission of s 5 (Forms) 4. Section 5— omit . ˙ Amendment of s 12 (Registration) 5.(1) Section 12(a)(iii), ‘Forms 3 and 4’— omit, insert — ‘the approved forms’. (2) Section 12(b), ‘(ii)—’— omit, insert — ‘(ii) either of the following documents—’. (3) Section 12(b)(ii)(A), ‘the Board; or’— omit, insert — ‘the board;’.
