Pharmacists (Amendment) Regulations 2001

The Governor in Council, on the recommendation of the
Pharmacy Board of Victoria, makes the following
Regulations:

1. Objective

The objective of these Regulations is to prescribe the form of an application for endorsement of registration for the purposes of section 16A of the Pharmacists Act 1974.

2. Authorising provision

These Regulations are made under section 37 of the Pharmacists Act 1974.

3. Commencement

These Regulations come into operation on

1 January 2002.

4. Amendment to the objectives

Pharmacists (Amendment) Regulations 2001

In regulation 102 of the Pharmacists Regulations
19921, after paragraph (a) insert—

"(ab) the form of application for endorsement

of registration of a pharmacist under

section 16A of the Act; and".

5. New regulation 202A inserted

After regulation 202 of the Pharmacists

Regulations 1992 insert—

"202A. Form of application for endorsement of

registration to practise Chinese medicine

For the purposes of section 16A(6)(a) of the
Act, the prescribed form is set out in

Schedule 1A.".

6. New Schedule 1A inserted

After Schedule 1 to the Pharmacists Regulations

1992 insert—

Pharmacists Regulations 1992

APPLICATION FOR ENDORSEMENT OF REGISTRATION UNDER SECTION 16A OF

THE PHARMACISTS ACT 1974

To the Pharmacy Board of Victoria
I apply for endorsement of registration under section 16A
of the Pharmacists Act 1974—

* to practise Chinese medicine.

* to obtain, have in my possession and to use, sell or
supply Schedule 1 poisons within the meaning of the
Drugs, Poisons and Controlled Substances Act 1981.
* I am a registered pharmacist under the Pharmacists Act
1974.

* I enclose a separate application for registration as a
pharmacist under the Pharmacists Act 1974.
