2 Pest Control Operators Amendment (No. 1) No. 106, 1994 ˙ Short title 1. This regulation may be cited as the Pest Control Operators Amendment Regulation (No. 1) 1994 . ˙ Regulation amended 2. This regulation amends the Pest Control Operators Regulation 1977 . ˙ Replacement of ss 2–5A 3. Sections 2 to 5A— omit, insert— ˙ ‘Forms ‘2.(1) If a provision of Division 4C of Part 4 of the Act or this regulation requires or permits a person to use a prescribed or approved form— (a) the Chief Health Officer must approve a form for the purpose of the provision; and (b) the form approved by the Chief Health Officer is the prescribed or approved form for the purpose of the provision. ‘(2) A person may ask the Chief Health Officer for a copy of a document setting out an approved form. ‘(3) The Chief Health Officer must promptly comply with the request. ˙ ‘Application for a licence ‘3.(1) An application for a licence must be made to the Chief Health Officer in the approved form. ‘(2) The application must be accompanied by the application fee set out in the Schedule. ˙ ‘Application for renewal of a licence ‘4.(1) An application for the renewal of a licence must be made to the Chief Health Officer in the approved form.
