2 Optometrists Amendment (No. 1) No. 503, 1993 ˙ Short title 1. This by-law may be cited as the Optometrists Amendment By-law (No. 1) 1993 . ˙ Commencement 2. This by-law commences on 1 January 1994. ˙ Amended by-law 3. The Optometrists By-law 1984 is amended as set out in this by-law. ˙ Replacement of ss.4 and 5 4. Sections 4 and 5— omit, insert— ˙ ‘Forms ‘4.(1) The prescribed forms for the purposes of the Act are the forms approved by the Board. ‘(2) A person may ask the Board for a document setting out an approved form. ‘(3) The Board must comply promptly with a request. ˙ ‘Fees ‘5. The fees payable for the purposes of the Act are set out in Schedule 1. ’. ˙ Replacement of ss.10 and 11 5. Sections 10 and 11— omit, insert—
