2 Occupational Therapists Amendment (No. 1) No. 507, 1993 ˙ Short title 1. This by-law may be cited as the Occupational Therapists Amendment By-law (No. 1) 1993 . ˙ Commencement 2. This by-law commences on 1 January 1994. ˙ Amended by-law 3. The Occupational Therapists By-law 1981 is amended as set out in this by-law. ˙ Amendment of s.4 (Fees) 4. Section 4(1)— omit ‘ in Schedule 1 ’, insert ‘ in the Schedule ’. ˙ Replacement of s.5 (Forms) 5. Section 5— omit, insert— ˙ ‘Forms ‘5.(1) The prescribed forms for the purposes of the Act are the forms approved by the Board. ‘(2) A person may request the Board to provide a copy of a document setting out an approved form. ‘(3) The Board must comply promptly with a request. ’. ˙ Amendment of s.11 (Registration) 6. Section 11(c)— omit ‘ in Form 6 of the Second Schedule ’, insert ‘ in the approved form ’.
