STATUTORY RULES.
REGULATION UNDER THE NATIONAL SERVICE ACT 1951-1957.*

THE GOVERNOR-GENERAL in and over the Commonwealth of Australia, acting with the
advice of the Federal Executive Council, hereby make the following Regulation
under the

Amendment of the National Service Regulations.
The Schedule to the National Service Regulations is amended by omitting Form 1 and inserting in its stead the following form:—

REGISTRATION FORM.
Note.—(i) You must answer each question: if it does not apply to you, write “Nil”.
(ii) You must also sign this form.
1. Surname (in block letters)
2. Christian names (in full)
3. Usual place of living, stating—
(
a ) the house number, street, and town or suburb, in which you live (Do not give an address care of a Post Office. )(
b )the Municipality, Shire, District Council Area or Roads Board District in which you live(
c ) your telephone number4. Present place of residence (if the same as your usual place of living, write “as above”, if different from your usual place of living, state the house number, the street, the town or suburb, the Municipality, Shire, District Council Area or Roads Board District, and the telephone number of your present place of residence)

11. If suffering from serious physical handicap, e.g., blind or crippled, or in receipt of an invalid pension, give particulars

d ) your usual occupation in that business or industry(
