REGULATION UNDER THE NATIONAL SERVICE ACT 1951-1957.*

I, THE GOVERNOR-GENERAL in and over the Commonwealth of Australia, acting with the advice of the Federal Executive Council, hereby make the following Regulation under the National Service Act 1951-1957.

Amendment of the National Service Regulations.

Amendment of the Schedule.

The Schedule to the National Service Regulations is amended by omitting Form 1 and inserting in its stead the following form:—

REGISTRATION FORM.

Note.—(i) You must answer each question: if it does not apply to you, write “Nil”.

(ii) You must also sign this form.

1. Surname (in block letters)

2. Christian names (in full)

3. Usual place of living, stating—

(a) the house number, street, and town or suburb, in which you live (Do not give an address care of a Post Office.)

(b)the Municipality, Shire, District Council Area or Roads Board District in which you live

(c) your telephone number

4. Present place of residence (if the same as your usual place of living, write “as above”, if different from your usual place of living, state the house number, the street, the town or suburb, the Municipality, Shire, District Council Area or Roads Board District, and the telephone number of your present place of residence)

5. Date of Birth—The day of , 19

6. Country of Birth (if Australia, give city or town, and State or Territory)

7. If not born in Australia, state date of arrival in Australia

8. Nationality

9. if you are a British subject, state—

(a) whether natural born or naturalized

(b) if naturalized, place and date of naturalization

10. If you are not a British subject, state number of your Alien Registration Certificate

11. If suffering from serious physical handicap, e.g., blind or crippled, or in receipt of an invalid pension, give particulars

12. State the following particulars about your employment:—

(a) employer’s name

(b) address of place where you work

(c) kind of business or industry in which you work

(d) your usual occupation in that business or industry

(e)are you an apprentice? ………………..(If you are attending a technical school or studying by correspondence in connexion with your apprenticeship, give details in question 13 below.)

(f) if you are temporarily unemployed, give particulars of your usual occupation and the industry in which you usually work

(g)if you have not yet commenced work but intend to do so in the near future give particulars of your proposed employment

13. State the following particulars about your education:—

(a) are you a student now? ………………………If “yes”, state —

(i) whether full-time, part-time, day, evening or correspondence

(ii) name of school, college or university

(iii) course of study

(iv) stage reached

(b) if you are not at present a student but intend to become one soon or if you intend to continue your studies after completing your present course, give full particulars of the proposed course, including name of school, college or university and whether full-time or part-time, and whether day, evening or correspondence

14. If you wish to do your training at a particular time of the year, state the months you prefer and your reason

15. (To be completed only by applicants for early registration.)I am seventeen years of age and wish to register early for the following reason
