I, SUSSAN LEY, Minister for Health, make this declaration under subregulation 59(3) of the National Health (Pharmaceutical Benefits) Regulations1960.

  1. Name of Declaration

    (1)   This declaration is the National Health (Medication chart prescription trial hospital) Amendment Declaration 2015.

    (2)   This declaration may also be cited as PB 54 of 2015.

  2. Commencement

    This declaration commences on 1 July 2015.

  3. Amendment

    This declaration amends the National Health (Medication chart prescription trial hospital) Declaration 2015 (PB 22 of 2015).

  4. Schedule One

4.1           Delete the following listed approved hospitals from Schedule 1.

Mackay Base Hospital (Mackay, Queensland)

Armadale Health Service (Armadale, Western Australia)

Bundaberg Base Hospital (Bundaberg, Queensland)

Bairnsdale Regional Health Service (Bairnsdale, Victoria)

Redland Hospital (Cleveland, Queensland)

Osborne Park Hospital (Stirling, Western Australia)

4.2           Insert the following listed approved hospitals into Schedule 1.

Mater Hospital Brisbane (South Brisbane, Queensland)

Mater Private Hospital Brisbane (South Brisbane, Queensland)

Monash Medical Centre, Clayton (Clayton, Victoria)

Jessie McPherson Private Hospital (Clayton, Victoria)

St Andrew's War Memorial Hospital (Brisbane, Queensland)

Royal Melbourne Hospital (Parkville, Melbourne)
