I, CHARLES MASKELL-KNIGHT, delegate of the Minister for Health and Ageing acting under paragraph (bj) of Schedule 1 to the National Health Act 1953 (the Act):

AMEND, with effect from 20 March 2005, the determination made under paragraph (bj) of Schedule 1 to the Act dated 30 June 1999 (the Determination), by OMITTING Schedule 4 of the Determination, and SUBSTITUTING the attached Schedule 4.
