Act to amend the
This Act may be cited as the
Consent to Medical Treatment and Palliative Care (Prescribed Forms) Amendment Act 2004 .
This Act will come into operation on a day to be fixed by proclamation.
In this Act, a provision under a heading referring to the amendment of a specified Act amends the Act so specified.
Part 2—Amendment of Consent to Medical Treatment and Palliative Care Act 1995
Section 4, definition of
dentist —delete the definition and substitute:
dentist means a person who is registered on the register of dentists or register of specialists under theDental Practice Act 2001 ;
5—Amendment of section 7—Anticipatory grant or refusal of consent to medical treatment Section 7(2)—delete "Schedule 2 or in a form prescribed by" wherever occurring
6—Amendment of section 8—Appointment of agent to consent to medical treatment
(1) Section 8(2)(a)—delete "Schedule 1" and substitute:
regulation
(2) Section 8(2)(b)—delete "or to the effect of the certificate in Schedule 1" and substitute:
prescribed by regulation or in a form to similar effect
Schedules 1 and 2—delete Schedules 1 and 2
