NOTICE OF PARTICULARS OF REVOCATION OF CONFORMITY ASSESSMENT CERTIFICATES UNDER SECTION 41EV OF THE THERAPEUTIC GOODS ACT 1989
I, Cheryl McRae, a delegate of the Secretary of the Department of Health for the purpose of Section 41EV of the Therapeutic Goods Act 1989 and acting under that provision, hereby publish the following particulars of revocations of conformity assessment certificates:
Under Section 41ES of the Therapeutic Goods Act 1989, the Secretary has, by written notice, revoked the following conformity assessment certificates:
On 19 September 2014 – the conformity assessment certificate held by Carern Marketing Pty Ltd – Conformity Assessment Certificate Number AU Q00269 – at the request of the holder of the certificate.
On 18 September 2014 – the conformity assessment certificate held by Sinclair Pharma SRL – Conformity Assessment Certificate AU Q00296 & AU D00309 - at the request of the holder of the certificate.
On 5 August 2015 – the conformity assessment certificate held by Qlicksmart Pty Ltd – Conformity Assessment Certificate AU Q00075/3 – at the request of the holder of the certificate.
On 5 August 2015 – the conformity assessment certificate held by Compumedics Limited – Conformity Assessment Certificate AU Q00106/3 – at the request of the holder of the certificate.
On 6 August 2015 – the conformity assessment certificate held by Sirtex Medical Limited – Conformity Assessment Certificate AU Q00119/4 & AU D00125/3 – at the request of the holder of the certificate.
On 6 August 2015 – the conformity assessment certificate held by Atcor Medical Pty Ltd – Conformity Assessment Certificate AU Q00138/2 – at the request of the holder of the certificate.
On 20 August 2015 – the conformity assessment certificate held by Ulco Engineering Pty Ltd – Conformity Assessment Certificate AU Q00052/5 – at the request of the holder of the certificate.
